How To Change Your Name on a Driving License
If you want to change your name on your driving licence you’ll only be able to do so via post.
Simply fill in a D1 order form (or Form D2, if you have a truck or lorry licence) and post it to DVLA with the same address as above. You will also need to submit any of the following original documents to the DVLA after changing your name:
-A marriage or civil partnership certificate
-A deed poll
-Your decree nisi or decree absolute, along with your birth or adoption certificate or UK certificate of naturalisation
-A statutory declaration
When changing your name and address, you will also need to provide the DVLA with your old driver’s licence that displays your former name.
Additional information – The same process applies if you want to change your name title (e.g. Ms. to Dr.). There is no need to send any evidence unless it is a hereditary title (e.g. titles of nobility).
